NJMMP Patient Program Summary

Speak to your primary physician about registering with the New Jersey Medicinal Marijuana Program (NJMMP). The program is part of the Department of Health and Senior Services (DHSS). Patients who wish to participate must register with DHSS online and receive a State-issued ID card.

Patient Qualifications:

1. You must be a New Jersey resident
2. Your doctor must be registered with the NJMMP
3. Your doctor must certify that you have a qualifying disease or condition

Qualifying Diseases and Conditions:
  • Chronic pain related to musculoskeletal disorders
  • Chronic pain of visceral origin
  • Migraine
  • Anxiety
  • Tourette’s Syndrome
  • Amyotrophic lateral sclerosis
  • Multiple sclerosis
  • Terminal cancer
  • Muscular dystrophy
  • Inflammatory bowel disease, including Crohn’s disease
  • Terminal illness, if the physician has determined a prognosis of less than 12 months of life.

The following conditions apply, if the patient is resistant to, or intolerant to, conventional therapy:

  • Seizure disorder, including epilepsy
  • Intractable skeletal muscular spasticity
  • Glaucoma
  • Post-Traumatic Stress Disorder

The following conditions apply, if severe or chronic pain, severe nausea or vomiting, cachexia or wasting syndrome results from the condition or its treatment:

  • Positive status for human immunodeficiency virus
  • Acquired immune deficiency syndrome
  • Cancer 

4. You must have a bona fide relationship with your certifying physician.

Bona Fide Doctor-Patient Relationship:
  • At least 1 year of care or…
  • At least 4 visits for the qualifying condition or…
  • You agree to treat the qualifying condition

5.  Your doctor must be registered in the program and must begin your registration, which you will complete at http://njmmp.nj.gov.

About Your Primary Caregiver:

You may also designate a primary caregiver Among other restrictions, the primary caregiver cannot serve in the same role for another patient and cannot be the patient’s physician. Primary caregivers must register with the State.

About Your Alternative Treatment Center:

When registering, you must select Compassionate SciencesTM or another state-recognized alternative treatment center as your designated dispensary of medicinal Cannabis. Registered patients can easily change their ATC, but may only be registered with one at a time.

Patient Registration:

Patients MUST register at http://njmmp.nj.gov. To do so, you need an e-mail address, a reference number provided by your physician and access to a computer or mobile device with internet access.

What Do I Need?

Your General Checklist:

  • Your full name, address, date of birth, telephone number and signature
  • Name, address and telephone number of your physician
  • Physician certification identification number (obtain from your doctor)
  • Name, address and telephone number of your alternative treatment center*
  • Patient-doctor relationship: at least 1 year of care; or at least 4 visits; or doctor takes responsibility to treat your condition after comprehensive examination

* Patients can register with only one alternative treatment center at a time

What Else Do I Need?

Your Document Checklist:

  • Proof of identity (one or more of the following):
    • New Jersey driver’s license
    • Government-issued photo ID card
    • Utility bill issued within previous 60 days
  • One recent, passport-sized color photograph
  • Caregiver photo ID and proof of New Jersey residency (if applicable)
  • For minors, written consent & accountability statement by parent, guardian or custodian
What Does it Cost?

Registration Fee:

The State will charge a registration fee of $100 (one hundred dollars) if you qualify for the NJMMP and receive a State-issued ID card.

Discounted Fee:

The State will charge a discounted registration fee of $20 (twenty dollars) if you participate in one of the following programs:

  • New Jersey Medicaid Program
  • New Jersey Temporary Disability Insurance
  • Supplemental Security Income (SSI)
  • Social Security Disability (SSD)
  • Food stamps (NJ SNAP)

Primary Caregiver Registration:

Primary Caregiver Qualifications:
  • You are a New Jersey resident
  • You are at least 18 years old
  • You agree to assist a qualifying patient
  • You are not providing care to another NJMMP patient
  • You are not the patient’s physician
  • You receive a criminal background check
  • You register online with the State at http://njmmp.nj.gov
  • You receive an ID card from DHSS

Note: the patient must initiate caregiver registration

  • Your full name, address, date of birth, telephone number and signature
  • Written consent to submit to a criminal background check
  • Application fee ($100)
  • Statement that you will assist with medical use of marijuana, are not caring for another patient and are not the qualifying patient’s physician
  • Proof of New Jersey residency (one or more of the following):
    • New Jersey driver’s license
    • Government–issued ID card with your name and address
    • Utility bill issued within previous 2 months
  • One recent, passport-sized color photograph
